Use of Topical Heparin in the Management of Acute Partial Thickness Burn Wound

Abstract
Backgrounds: Wound healing after burn injuries are accomplished by a complex phenomenon between epithelial cells, extra-cellular matrix and inter-cellular signals induced by burn injuries. Many reports have suggested that control of infection and inflammation, balance of moist wound healing environments and growth factors and cytokines, systemic or local heparin promote faster healing of partial thickness burn wound. It is well known that heparin has antithrombolytic and anti-inflammatory actions, and it brings reducing of edema and pain. It has been postulated the it could prevent burn wound conversion from superficial burn to deep burn. in partial thickness burn. Therefore use of heparin could promote more rapid wound healing in partial thickness burn. We have newly made the acute burn wound dressing modality of effective topical applications of heparin, which is soaked in the hydrophilic wound healing product. And we call it as heparin hydrodressing. This study reports clinical experiences of acute burn wounds treated with topical heparin hydrodressing.

Methods: 13 patients with partial thickness burn wound treated with topical heparin. In cases of burn wound, which showed the excoriation of bullae, topical heparin dressing was applied in a fashion of heparin 100 U/ml solution soaked in the hydrophilic dressing products such as AquaCel?or Mediform? [Heparin hydrodressing]. In some cases, the Tegaderm?or Op site?were overdressed for more long maintaining of moist condition of dressing site. The heparin hydrodressing changes were done in a interval of 1 or 2 days with same manner. The results assessments were done by healing time, pain, complications by topical heparin use, and appearance of healed burn sites after follow ups.

Results: We have experienced the managements of 12 cases of partial thickness burn over mid 2nd degrees, which had exocoriated bullae from the burn wound, and 1 case of 3rd degree burn using heparin hydrodressing method. 8 of the patients were male and three female. Patient¡¯s age were ranged between 8 months and 49 year-old. The causes of burn were 9 scalding burn, 3 flame burns, and 1 electric flash burn. The time to heal is mean 12.8 days in 5 cases of mid 2nd degree burns, and the healing time of 7 cases mid to deep 2nd degree burns ranged from 12 days to 32 days. In one case of 3rd degree burns, there were marked reduction of operation sites after its use. The results showed that topical heparin brings more rapid healing of mid to deep second degree burn wound than the expected healing date. There was no pain during its use, and there was no complications. After follow-up of healed partial thickness burn wound site, there were relatively good appearances in all cases.

Conclusions: In the management of acute partial thickness burn wounds, the wounds treated with topical heparin hydrodressing showed more rapid wound healing, and showed relatively good appearance of healed burn sites after follow ups. Further studies for its clinical use should be mandatory. (Journal of Korean Burn Society 2007;10:118-125)
